About this form

Use this form to pay for your DOCD or DPP (MMS-137, MMS-139). Fee is calculated based on the number of wells.

If necessary, provide the following information to your bank to prevent debit blocking on the payments processed through Pay.gov: ACH Company ID =1419000123

• Debit or credit card
