This form is to pay fees for formal written notices filed with the Committee on Foreign Investment in the United States (CFIUS). The information provided is subject to 31 CFR 800.802/31 CFR 802.802. NOTE: Filing fees are only required for formal written notices submitted to CFIUS under 31 CFR 800.501(a) or 31 CFR 802.501(a)- filing fees are not required for declarations filed under 31 CFR part 800, subpart D, or 31 CFR part 802, subpart D.
