About this form

Every DOI employee who submits an Official Passport Application must also submit their agency's payment of $158.90 via government purchase card for processing the application, using this form. All payments collected will be transferred to the State Department's Special Issuance Agency, which beginning in FY17, bills federal agencies to process Official Passport Applications.
