What this form is for: Use this form to pay filing fees in the United States Bankruptcy Court for the Southern District of Ohio, including installment payments and fees relating to searches, copies, certifications, and recordings of hearings.

What this form is not for: Do not use this form for Chapter 13 plan payments, which should be paid directly to your trustee, or for fees relating to motions to appear pro hac vice, which should be paid directly to the District Court.
