Find a Form or Agency
You must use the correct payment form, or your payment will not be credited by the agency you wish to pay.
Different payment forms may have similar names and descriptions, but it is important to choose the correct form. Do not guess which form to use.
If you are unsure which form to use, contact the agency you are paying. See below for more information about how to find the correct payment form.
The most common payment forms, including the VA Medical Care Copayment form, are on Pay.gov's home page.
You can also use the What would you like to do? section at the bottom of the home page, located below the most common forms.
Click "See all forms" to see a listing of all forms by form name or for each agency. You can see all the forms for any agency by clicking a checkbox next to one or more agency names or use the "Filter" box to search by typing in a search term (for example, "Coast Guard").
You can also use the search box at the top of most Pay.gov pages. Enter one of the following and then click search:
- All or part of the agency name
- All or part of a payment form name or title
- All or part of a form description
- A word about what you are looking for (park, loan, training, fish, etc.)
- The form's OMB or agency number
